VASOMIL INJECTION

COMPOSITION:
Each 2 mL ampoule contains
Verapamil hydrochloride 5 mg.

PHARMACOLOGICAL CLASSIFICATION:
A 7.1.4 Vasodilators –Coronary and other medicines used in angina pectoris.

PHARMACOLOGICAL ACTION:
Verapamil, a calcium antagonist, reduces the workload on the heart by decreasing peripheral resistance.

INDICATIONS:
Verapamil injection is used in patients with angina pectoris who do not respond to oral verapamil. Verapamil may be given intravenously in the treatment of supraventricular arrhythmias.

CONTRA-INDICATIONS:
Verapamil is contra-indicated in cardiogenic shock, the acute stage of cardiac infarction, and complete heart block. It is also contra-indicated in uncompensated heart failure.
Verapamil should be given with care intravenously in patients receiving digitalis as complete heart block may be precipitated
Verapamil should not be injected with or immediately after therapy with beta-adrenergic blocking agents.

DOSAGE AND DIRECTIONS FOR USE:
Verapamil is used by slow intravenous injection in doses of 5 mg up to thrice daily in severe angina of effort not responsive to oral therapy. The patient’s condition being monitored electrocardiographically. Injection of verapamil should be discontinued as soon as the desired effect is observed.
In arrhythmias, 5 - 10 mg slowly intravenously.
ECG Control is necessary.

SIDE-EFFECTS AND SPECIAL PRECAUTIONS:
Nausea, vomiting, dizziness, flushing and hypotension may occur. Verapamil may reduce blood pressure in patients with hypertension and a reduction in antihypertensive therapy may be needed. Because it reduces cardiac output it may aggravate or precipitate cardiac failure. Verapamil should be used with extreme caution in patients with partial heart block bradycardia, or hypotension. Allergic reactions may occur. Verapamil may cause a complete A-V block and even asystolia after intravenous injection.

KNOWN SYMPTOMS OF OVERDOSAGE AND PARTICULARS OF ITS TREATMENT:
Severe cardiac depression and hypotension may occur. Heart block treatment is symptomatic and the effects of verapamil may be reversed by the administration of adrenaline or orciprenaline by intravenous infusion.
